INSURANCE
It is very important for you to be aware of the requirements of your health plan. Our staff tries to be helpful, but cannot know everything about the hundreds of insurance plans available to our patients.
If you have any questions about your benefits, call your insurance company service department (the telephone number is usually listed on the back of the card) or contact your employee’s human resource department.
PARTICIPATION AND FILING
We will submit to your primary and secondary insurance (two companies only).

PAYMENTS
In order to keep costs down
we accept only cash and checks. Payment is expected at time of service
unless other arrangements have been made.
RECORDS RELEASE
Patients
may request copies of their records or summary of treatment by
completing a medical release form. This form is available at this office
or at the office you want the records sent to. There is no charge for
copies.
We require 72 hours notice to prepare and copy records and X-rays.
We
are ready to assist you to complete any other benefit forms needed.
Please be sure to fill out all of your personal information prior to
giving the forms to the doctors. We need at least 72 hours to complete
theses forms.
